By submitting this form, the Client agrees to enter all details (rental duration, equipment list, cost, replacement value, and delivery terms) exactly as provided in the accompanying Rental Terms Summary issued by Your Unforgettable. Any discrepancies between the form submission and the Rental Terms Summary may render this agreement invalid at the sole discretion of Your Unforgettable. The Rental Terms Summary, together with this signed form, constitutes the full agreement.

Terms & Conditions:

1. Use of Equipment: The Client agrees to use the rented equipment in a safe and responsible manner and only for its intended purpose.
2. Condition of Equipment: All equipment is provided in good working condition. It is the Client’s responsibility to inspect and report any issues at the time of delivery or pickup.
3. Client Responsibility: The Client assumes full responsibility for all rented equipment from the time of delivery/pickup to return. The Client agrees to pay the full replacement cost, as listed above, for any equipment that is lost, stolen, or damaged beyond repair during the rental period.
4. Return of Equipment: Equipment must be returned on or before the agreed-upon end date in the same condition in which it was received, excluding normal wear and tear.
5. Payment Terms: Full payment, including any applicable security deposit, is due prior to or upon delivery/pickup unless otherwise agreed in writing.
6. Indemnification: The Client agrees to indemnify and hold harmless Your Unforgettable from any claims, liabilities, damages, or injuries arising from the use of the rented equipment.
7. Governing Law: This Agreement shall be governed by the laws of the State of Kansas.
8. Entire Agreement: This document constitutes the entire agreement between the parties and supersedes all prior discussions, agreements, or understandings of any kind.
